- Identification of the copyrighted work that you claim has been infringed
- Identification of the material that is claimed to be infringing and where it is located on our website or services
- Information reasonably sufficient to permit us to contact you, such as your address, telephone number, and email address
- A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or law
- A statement, made under penalty of perjury, that the above information is accurate and that you are the copyright owner or are authorized to act on behalf of the owner

Counter-Notification
If you believe that material you posted was removed or access to it was disabled by mistake or misidentification, you may file a counter-notification with our Copyright Agent by providing the following information in writing:
- Your physical or electronic signature
- Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access was disabled
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification
- Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the courts in your location and that you will accept service of process from the person who provided notification of the alleged infringement
If a counter-notice is received by our Copyright Agent, we may send a copy of the counter-notice to the original complaining party informing that person that we may replace the removed material or cease disabling access to it. Unless the copyright owner files an action seeking a court order against the user, the removed material may be replaced or access to it restored within a reasonable timeframe.

We may also, at our sole discretion, limit access to our website and services or terminate the accounts of any users who infringe any intellectual property rights of others, whether or not there is any repeat infringement.
